****Case Manager (LVN) – Full-Time**** **Facility:** Vasona Creek Healthcare Center **Salary:** $90,000 – $100,000 / year (Depending on experience) **Job Type:** Full-time **Job Summary** Vasona Creek Healthcare Center is seeking a dedicated and organized Licensed Vocational Nurse (LVN) to join our team as a Case Manager. In this role, you will coordinate patient care, manage utilization reviews, and oversee discharge planning to ensure high-quality outcomes.
