Job Description

**Your Role as Case Manager – Lockyer Valley Housing Service** The Case Manager, Lockyer Valley Housing Service (LVHS), supports individuals and families experiencing or at risk of homelessness through assessment, case management, advocacy, service coordination and referrals. Working across centre-based, outreach and remote service delivery, this role helps clients secure and sustain stable housing while fostering independence and connections to community supports. Key responsibilities of this role include: * Assessing eligibility, needs and risks of individuals and families experiencing or at risk of homelessness. * Providing holistic case management, including support planning, advocacy and tenancy sustainment assistance. * Coordinating services and referrals to ensure clients can access the support they need. * Build and maintain effective relationships with community partners, service providers and stakeholders. * Delivering a high-quality, customer-focused service that places client needs at the centre of all interactions. * Maintaining accurate case notes, client records, reports and program data in line with organisational requirements. * Working collaboratively within a multidisciplinary team and participate in meetings, supervision and professional development. * Supporting a safe, inclusive and values-driven workplace, including mentoring volunteers and student placements when required. **What We Offer** * **Salary Packaging:** As a not‑for‑profit employer, we offer generous salary packaging benefits that help maximise your take‑home pay.
