Our **Hospital in the Home (HITH)** service enables patients who would otherwise require hospital admission to safely receive acute-level treatment in their homes, residential aged care facilities, workplaces, or via virtual care technologies.

**About the Opportunity**
We're seeking experienced **Clinical Nurses** to join our expanding **Hospital in the Home (HITH)** service across Victoria.
As a HITH Clinical Nurse, you'll deliver advanced clinical care to patients with acute and complex healthcare needs while working as part of a progressive multidisciplinary team committed to innovation, compassion, and clinical excellence.
You'll use your advanced nursing expertise, clinical judgement, and care coordination skills to help patients receive safe, high-quality care in the comfort of their own homes.
**Key Responsibilities**
* Conduct comprehensive patient assessments and clinical reviews.
* Develop and implement individualised care plans.
* Deliver advanced nursing interventions including IV therapies and complex clinical care.
* Monitor patients with acute and complex healthcare needs and escalate care when required.
* Coordinate referrals, admissions, and discharge planning.
* Collaborate closely with Medical Officers, Clinical Leads, and multidisciplinary teams.
* Support patients, families, and carers through education and care planning.
* Participate in quality improvement initiatives and service development activities.
* Mentor and support new and junior team members.
* Contribute to innovative healthcare delivery that strengthens the connection between hospital and home.
**About You**
You're an experienced Registered Nurse who thrives in an autonomous environment, enjoys building meaningful patient relationships, and is passionate about delivering exceptional clinical care in community settings.
**You Will Bring**
* Current registration as a Registered Nurse with AHPRA.
* Minimum three (3) years post-graduate nursing experience.
* Experience in community nursing and/or acute medical, surgical, emergency, or critical care settings.
* Demonstrated experience supporting patients with complex clinical needs and multiple co-morbidities.
* Strong clinical assessment, care planning, and decision-making capabilities.
* Excellent communication and stakeholder engagement skills.
* Confidence working independently while collaborating with multidisciplinary teams.
* Strong digital literacy and willingness to utilise telehealth and virtual care technologies.
* Current unrestricted Victorian Driver's Licence.
* Full and unrestricted working rights in Australia.
**Highly Regarded**
* Hospital in the Home (HITH), community nursing, outreach nursing, or virtual care experience.
* Postgraduate nursing qualifications.
* Experience contributing to service innovation, quality improvement, or clinical projects.
