Family & Community Support Practitioner

**Your Role as Family & Community Support Practitioner** As a Family & Community Support Practitioner with Hummingbird House, you’ll make a meaningful difference in the lives of children and families facing serious illness, end-of-life care and bereavement. Working within a multidisciplinary team, you will provide compassionate psychosocial and emotional support, deliver family-centred interventions, and help families build resilience and wellbeing through challenging times. Key responsibilities include: * Conduct psychosocial assessments and develop tailored support plans for children and families. * Provide emotional, psychosocial, grief and bereavement support through individual, family and group interventions. * Support families during periods of crisis, acute distress and significant clinical events. * Facilitate grief and bereavement programs, therapeutic groups and memory-making activities. * Collaborate with multidisciplinary teams to deliver coordinated, family-centred care. * Build strong relationships with healthcare providers, schools and community organisations to support positive outcomes. * Provide information, referrals and practical support to children, young people and their families. * Maintain accurate records and deliver services in line with professional standards, legislation and organisational policies. _This is a part-time position consisting of 30 hours per week._ **What We Offer** * **Salary Packaging:** As a not‑for‑profit employer, we offer generous salary packaging benefits that help maximise your take‑home pay. You can package up to **$15,900** each year on everyday living expenses and up to **$2,650** on meals and entertainment. * **Wellbeing Support:** Access to discounts on private health insurance and gym memberships, and free, confidential counselling through our employee assistance program.  * **Team Culture**: Be part of a collaborative, multidisciplinary team across a diverse clinical setting that values your skills and wellbeing. **What We’re Looking For** * Completed tertiary qualification in Social Work and eligibility for AASW Membership. * Experience supporting children, young people and families through serious illness, palliative care, end-of-life care, grief and bereavement using evidence-based psychosocial interventions. * Strong knowledge of paediatric palliative care, childhood grief, child development and family-centred, trauma-informed practice, with the ability to undertake psychosocial assessments and care planning. * Ability to work collaboratively and within professional and ethical boundaries as part of a multidisciplinary team, including providing consultation, support, guidance and education to colleagues and stakeholders. **A Valuable Addition, Yet Not a Must-Have!** * Experience in planning, implementing and evaluating programs for children and young people of varying ages. * Knowledge of child development and articulates the value of play and psychosocial support for children affected by a life limiting condition; their siblings and other children or young people connected to the guest at Hummingbird House. * Postgraduate study, certification or specialist professional development in grief and bereavement, child and family mental health, paediatric palliative care counselling, social work or related disciplines. **Essential Requirements** * Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy) * Relevant probity checks required by legislation and WMQ Limited policy. * Current Driver’s Licence and willingness to travel for work **Apply Now!** Be part of something meaningful - Click Apply today!
