Learn more about the model at [www.cmocares.org](https://www.cmocares.org/?utm_source=copilot.com). **What You’ll Do** **Clinical Facilitation & Supervision** * Facilitate and/or participate in clinical meetings with staff, families, and/or ACS * Facilitate family meetings, assess clinical needs, and make referral recommendations * Direct supervision of staff; consultation to staff and colleagues as needed; development and delivery of trainings (mental health, substance use, and related topics) **Team & Program Management** * Oversee referrals, assign cases, and ensure timely completion of clinical documentation * Work closely with ACS, community providers, and internal teams to coordinate care * Monitor key deliverables, charting requirements, and program quality indicators **Schedule & Requirements** * Location: 334 E. 148th Street, Bronx NY 10451 (some travel may occur) * Hours: Monday–Friday, 8:30am–5pm; occasional evenings/weekends; periodic on‑call coverage * Spanish proficiency is a plus **Who Thrives in This Role** We’re looking for a clinician who brings: * A master’s required, doctoral degree preferred (Psychology, Social Work, Mental Health Counseling) * Active NYS License required (LCSW, LMSW, LMHC; licensed or limited permit Psychologists) * Experience working with families and/or child welfare systems * Strong clinical assessment skills (mental health, substance use, family functioning) * Excellent organizational and managerial abilities * A collaborative, strengths‑based mindset **Why Join Montefiore & UBA** Montefiore Medical Center is the largest employer in the Bronx, with 200+ locations and a deep commitment to community health.
