Performs duties and responsibilities in a manner consistent with mission, values and mercy service standards. **Position Details:** ===================== Additional Job Description **Remote Mental Health Case Manager, Mercy Virtual Collaborative Care** ----------------------------------------------------------------------- **Join a virtual collaborative care team where learning, evidence-based practice and consistent communication drive better outcomes. This role will provide case management/therapy for patients and families to enhance their psychosocial functioning.**   * Ensures safety, confidentiality, regulatory compliance, and adherence to hospital and infection control standards. * Coordinates interdisciplinary care, advocates for patients and families, and oversees safe, comprehensive discharge planning. * Provides behavioral health case management, assessments, therapy, crisis intervention, and ongoing treatment planning with accurate documentation. * Communicates effectively with patients, families, and community partners while maintaining strong clinical, organizational, and technology skills.   Qualifications:   * **Education:** Master's degree required in Social Work, Counseling or another human-service related field. * **Licensure:** Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) in Missouri required. Only a Licensed Clinical Social Worker (LCSW) is accepted at Mercy Hospital Perry. * **Experience:** Minimum 2-3 years Behavioral Health experience preferred. * **Preferred Other:** Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills.
