**Summary** **Job Description:** **Physical Therapist** **Full Time, First Shift** The ​Physical Therapist I shall evaluate and treat patients; develop a treatment plan; and set goals for patients.  Patient and family education is included in the treatment plan and goals. **Job Duties:** * ​​Completes appropriate functional evaluation including assessment of pain for various musculoskeletal/neurological conditions. * ​Plans treatment appropriately including input from patient and/or caregivers.   * ​Provides services to improve functional independence and/or reduce pain such as: therapeutic procedures/exercises or activities to promote ROM, strength, coordination, balance, functional mobility (bed mobility, transfers, floor transfers), ADL training, gait training, balance and vestibular training, education, and use of assistive devices.   * ​​Receives and implements therapy orders.  * ​Completes evaluation form, treatment notes, progress reports and D/C summaries in a timely manner and appropriately submits to referring provider.  **Requirements:** * **​​**Bachelors in Physical Therapy​  * ​​Eligible for a temporary license or Licensed as a Physical Therapist by the Kentucky Board of Physical Therapy ​  **Preferred Requirements:** * Acute Care **Benefits:** * Tuition Assistance reimbursement program * Paid Time Off available for use upon hire * Company paid Maternity and Paternity Leave * Bereavement Leave (includes pets) * Employee Assistance Program * Retirement plans with company match ### _If you would like to be part of a growing family focused on supporting clinical excellence, teamwork and innovation, we urge you to apply now!_ ### Baptist Health is an _Equal Employment Opportunity_ employer.
