You will collaborate with physicians, families, and interdisciplinary team members to provide high-quality, patient-centered care. ### **Key Responsibilities** * Perform **patient evaluations** and develop individualized treatment plans * Provide therapy to improve **strength, balance, mobility, and overall function** * Monitor patient progress and adjust treatment plans as needed * Maintain accurate and compliant **clinical documentation** * Communicate with physicians, residents, families, and care team members * Participate in **care planning, rehab meetings, and discharge planning** * Conduct **staff education and in-service training** * Supervise **PTAs, aides, and students** ### **Qualifications** * Degree in **Physical Therapy** (Bachelor’s required; Master’s or Doctorate preferred) * Active, unencumbered **Physical Therapist license in South Carolina (required)** * Must maintain all continuing education and remain in good standing with the State Board * Experience in a **skilled nursing or rehab setting preferred** * Familiarity with **PointClickCare (PCC) and Casamba preferred** * Strong clinical, leadership, and communication skills ### **Why Join Fleetwood Post Acute?** At Fleetwood, you’ll be part of a **stable, experienced team** that values your expertise and supports your growth.
