You will play a key role in helping patients regain independence and achieve personalized therapy goals. ### **Key Responsibilities** * Perform **patient evaluations** and develop individualized treatment plans * Deliver therapeutic interventions to improve strength, balance, and mobility * Monitor patient progress and adjust treatment plans as needed * Maintain accurate and timely documentation * Collaborate with interdisciplinary teams to support patient outcomes * Educate residents and families on therapy techniques and recovery goals ### **Qualifications** * **Bachelor’s degree required**; advanced degree preferred * Active, unencumbered **Physical Therapist license in South Carolina** * Experience in a **long-term care or rehab setting preferred** * Familiarity with **PointClickCare (PCC) and Casamba** preferred * Strong communication and clinical skills * Ability to work independently and as part of a team **Why Join Chandler Creek Post Acute?** --------------------------------------- Join a team where your skills are valued and your work truly makes a difference.
