University Health is seeking a **Population Health Intervention Specialist – RN** to lead innovative programs that improve health outcomes, reduce barriers to care, and empower individuals to successfully manage chronic conditions.

### What You'll Do
### As a Population Health Intervention Specialist, you will serve as a clinical leader supporting patients and communities through education, care coordination, and evidence-based interventions. You'll partner with healthcare providers, community organizations, and interdisciplinary teams to improve access to care and help patients achieve healthier outcomes.
### In this role, you will:
* ### Lead evidence-based chronic disease prevention and self-management programs focused on conditions such as diabetes, hypertension, cardiovascular disease, obesity, and other high-risk populations.
* ### Conduct health risk assessments and identify opportunities to improve prevention, medication adherence, self-management, and patient engagement.
* ### Provide individualized health coaching using motivational interviewing, trauma-informed communication, and behavior change strategies.
* ### Guide patients through the healthcare system by connecting them with clinical services, community resources, and social support programs.
* ### Build strong partnerships with community organizations, public health agencies, healthcare providers, and social service organizations.
* ### Support initiatives addressing health disparities, food insecurity, preventive care, and other social drivers of health.
* ### Coordinate community education programs, health screenings, and wellness events that improve community health.
* ### Collaborate with operational and clinical leaders to achieve quality, utilization, and population health goals.
* ### Assist with grants, pilot programs, and special population health initiatives.
* ### Monitor program outcomes, quality measures, and utilization trends to drive continuous improvement.
* ### Maintain accurate documentation and ensure compliance with organizational and regulatory standards.
* ### Champion a culture of quality, patient safety, health equity, and compassionate, community-centered care.
### What You'll Bring
### Required Qualifications
* ### Bachelor of Science in Nursing (BSN)
* ### Current Missouri Registered Nurse (RN) license in good standing
* ### Five (5) or more years of experience in population health, community health, chronic disease management, care coordination, program management, or a related healthcare setting
* ### Strong understanding of population health, public health principles, health equity, and social drivers of health
* ### Experience providing patient education, health coaching, motivational interviewing, and interdisciplinary care coordination
* ### Excellent communication, presentation, and relationship-building skills
* ### Strong organizational skills with the ability to manage multiple priorities and community partnerships independently
* ### Proficiency with Microsoft Office and healthcare documentation systems
* ### Ability to work occasional evenings and weekends for community outreach and engagement activities
### Preferred Qualifications
* ### Bilingual (Spanish/English)
* ### Certified Diabetes Care and Education Specialist (CDCES)
* ### Community Health Worker (CHW), Certified Health Education Specialist (CHES), Health Coach, Lifestyle Medicine, or related certification
* ### Experience supporting value-based care initiatives, population health analytics, or community-based care models
