This is more than just a job – it’s a chance to build meaningful relationships and create lasting change.  **About the Role** This is an exciting opportunity to join Forward Leeds as a Recovery Coordinator within our Shared Care Team, based across York Street Health Practice and the Forward Leeds Kirkgate Hub. York Street Health Practice provides specialist primary healthcare for people experiencing homelessness and those living in vulnerable or unstable housing. In this unique role, you'll support adults with substance use needs alongside complex physical health, mental health and social challenges, helping them access treatment and build sustainable recovery. Working primarily from the office across both sites, you'll become an integral part of a multidisciplinary team, collaborating closely with GPs, nurses, mental health practitioners, housing providers and partner agencies. You'll also undertake occasional outreach visits to hostels, supported accommodation and other community settings to engage people who may struggle to access traditional services. No two days are the same. You'll combine clinical recovery work with assertive engagement, advocacy and practical support, helping people overcome barriers, reduce harm and move towards improved health, stability and independence. **What You'll Do** * Manage a varied caseload of adults with complex needs, developing and reviewing personalised recovery and treatment plans. * Complete triage, screening and comprehensive assessments, identifying risks and safeguarding concerns while delivering person-centred interventions. * Build trusted relationships with people who may have experienced homelessness, trauma, social exclusion and multiple disadvantage. * Work alongside GPs, practice staff and partner agencies to deliver truly integrated care through shared assessments, joint working and coordinated support. * Support individuals to access prescribing, healthcare, housing, benefits, mental health services and wider community resources that strengthen recovery and wellbeing. * Carry out occasional outreach to hostels and community settings to improve engagement with people who may otherwise find services difficult to access. * Accurately record interventions, risk assessments, safeguarding activity and recovery plans using SystmOne and other case management systems. * Promote recovery, independence and positive change, recognising that progress looks different for every individual. **What You'll Bring** We're looking for someone who is compassionate, resilient and motivated by working with people facing some of the greatest health and social inequalities. **You'll ideally have:** * Experience of managing a caseload within health, social care or substance use services. * Knowledge or experience of supporting people affected by alcohol and/or drug use. * Experience working with people who have multiple and complex needs, including homelessness, poor mental health, offending, trauma or social exclusion. * Excellent communication skills and the ability to build positive relationships with service users and partner agencies. * Confidence working both independently and as part of a multidisciplinary team. * A relevant Health and Social Care qualification (or equivalent experience).
