Accountable for the assessment, coordination delivery and evaluation of nursing care, including direct patient care, patient/family education and transitions of care. Develops, implements, manages/coordinates an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors.
