We are seeking a dedicated **Speech-Language Pathologist (SLP)** to join our full-time rehab team. You’ll collaborate closely with physicians, nursing, and therapy teams to deliver individualized, patient-centered care. ### **Key Responsibilities** * Perform **evaluations within 24 hours (or 1 business day)** of physician referral * Develop and implement **individualized treatment plans** with physician approval * Provide therapy services in accordance with treatment plans * Monitor patient progress and adjust care as needed * Maintain accurate clinical documentation (evaluations, progress notes, discharge planning) * Participate in **care plan meetings, utilization review, and interdisciplinary conferences** * Educate residents, families, and staff on therapy programs and techniques * Supervise **SLPAs and Clinical Fellows (CFY)** as applicable * Conduct **staff training and in-service education** * Ensure compliance with all regulatory and facility standards ### **Qualifications** * Master’s or Doctoral degree in **Speech-Language Pathology** * Active, unencumbered **SLP license in South Carolina (required)** * **CCC-SLP preferred** * Experience in a **skilled nursing or rehab setting preferred** * Knowledge of **speech, language, cognitive, and swallowing disorders** * Experience with **PointClickCare (PCC) and Casamba preferred** * Strong clinical, communication, and documentation skills * Ability to work both independently and as part of a team ### **Why Join Chandler Creek Post Acute?** At Chandler Creek, you’ll join a compassionate and collaborative team where your expertise is valued and your growth is supported.
