You will play a key role in promoting healing, preventing complications, and improving overall resident outcomes. ### **Key Responsibilities** * Provide **direct wound care and skin treatments** in accordance with physician orders * Assess, monitor, and document wound progress and treatment effectiveness * Assist in developing and **modifying individualized care plans** * Collaborate with the interdisciplinary team to ensure optimal outcomes * Educate staff on proper wound care techniques and prevention strategies * Maintain accurate and compliant clinical documentation * Ensure adherence to all regulatory, safety, and infection control standards ### **Qualifications** * Active, unencumbered **LPN license in South Carolina** * Graduate of an accredited **Practical Nursing program** * **Current CPR certification required** * Ability to work **12-hour shifts** * Experience with **PointClickCare (PCC) preferred** * Prior wound care experience strongly preferred * Strong clinical judgment and attention to detail * Passion for delivering compassionate, resident-centered care ### **Why Join Chandler Creek Post Acute?** At Chandler Creek, you’ll be part of a close-knit, supportive team where your specialized skills are valued and your growth is encouraged.
